In our Caribbean cosmetic surgery practice at Trinidad Institute of Plastic Surgery we use the Brava(TM) external expansion system and micro fat grafting for autologous breast augmentation with fat grafting and breast enhancement.
The external expansion system has to be worn over the breasts for at least eight hours per day for at least three weeks.
Thereafter a liposuction for fat harvest is performed in areas such as abdomen, thighs, flanks and hip rolls to obtain fat for transfer. The expansion by that time has created increased looseness of the breast tissues and thus space for the fat. Injection is performed in the area between breast tissue and chest wall muscle and in the spaces between the breast tissue and skin. Breast tissue itself is not injected with fat during breast augmentation with fat grafting.
No incisions need to be made anywhere on the breasts and the liposuction for fat harvest is done through the usual 4-5mm stab incisions.
Recovery after breast augmentation with fat grafting is also comparable as for a liposuction.
Best candidates for breast augmentation with fat grafting are self motivated clients able to “stick with it”, as the external expansion process is self directed after initial consultation with the surgeon. Sufficient body fat to harvest at least 1000ml of pure fat needs to be available.
